How much does this program cost?
There is no cost to participate in the program for eligible participants. Certain third-party medical costs, which include blood work, wearable continuous glucose monitors, medication prescriptions, and supplements will be billed through your insurance. The Milwaukee Bucks Foundation will cover any additional cost that your insurance does not cover. Costs that are not covered through the program include food and other associated grocery costs.
Can I eat out or will I need to make my own food during this program?
The HMW program encourages following the outlined nutrition program. You may eat out, however we recommend trying to choose foods that mimic the portions and foods outlined in your meal plan.
Will my meals be restricted?
Your food portions are outlined in your meal plan and are designed to provide you with nutrients to support your body throughout the day. Your meal plan should not feel restrictive, however some meal portions may be smaller than the portions you are used to consuming. If your plan feels too restrictive, please contact your HMW healthcare practitioner through the mobile app. to adjust your meal plan.
What if I get sick and miss an appointment?
If you are sick and unable to make it to your appointment, please let your HMW healthcare practitioner know via chat feature in the app as soon as possible. They will reschedule your virtual appointment for when you feel better.
How long are each of the workouts?
Workouts on the HMW program range from 10-30 minutes, depending on the day and pace of your workout.
How often am I expected to workout?
Exercise is a critical component of the program. Workout regimens are highly individual, but 3 times per week is typical.
Will I have a personal trainer?
You will not have access to a personal trainer throughout the program. The app provides your with an instructional video on how to complete each exercise. If you do not feel comfortable with any of the exercises, please let your HMW healthcare provider know via the chat feature in the app.
It is important to note, you know your body best! So if an exercise doesn’t feel right, don’t do it and let your HWM team know that it’s not a good exercise for your body.
Do I need to travel for any group exercise?
No, all exercise can be done from the comfort of your own home.
Are there additional travel expectations or check in’s I need to plan for?
Yes. All participants will be expected to travel to Gee’s Clippers at 2200 N. Dr. Martin Luther King Jr. Dr, once a month for the first three months for weigh-in and blood work. Then once every two months following that.
Are program check in times flexible, allowing after hours support?
Yes, virtual follow up appointments are flexible and typically available Monday through Friday between the hours of 9-5pm. If you have an after hours question or need support, you may use the chat feature in the app to reach out to your HMW healthcare practitioner. In person weigh-ins and blood work will be held during specific dates and times that will occur a few times a month to accommodate all participants busy schedules.
How long is this program?
The program is planned to last 12 months.
What are the typical results of individuals who follow this program?
Results may vary based on the individual and level of commitment to the program.
Who is on my HMW care team?
Your HMW care team is composed of a Physician, who is a board-certified Medical Doctor, a Registered Dietitian, and a Patient Care Coordinator, all provided by HMW Method, S.C.
How often will I meet with my HMW care team and how long are those visits?
You will meet with your care team every month for the first three months in the program, and then every two months following that. This initial session should take around 2 hours and follow-up appointments are around 30 minutes for each provider.
What happens if I can no longer participate in the program?
If you are unable to continue with the program after starting, please contact your HMW healthcare practitioner.
Are there additional travel expectations or check in’s I need to plan for?
Yes. All participants will be expected to travel to Bucks Health and Wellness at 101 W. Pleasant St, once a month for the first three months for weigh-in and blood work. Then at 6, 12, 18 and 24 months following that.
How long is this program?
The program is planned to last 12 months with follow up visits at 18 and 24 months.
How often will I meet with my HMW care team and how long are those visits?
You will meet with your care team every month for the first three months in the program, and then via telehealth every month following that. This initial session should take around 1 hour and follow-up appointments are around 30 minutes for each provider.
